fa ضرورت آموزش مستمر مستندسازی داده‌های پزشکی در مراکز آموزشی_درمانی: نگاهی به نقش چرخه آموزش، ارزیابی و بازخورد The Need for Continuous Education in Medical Documentation in Teaching Hospitals: The Role of an Education–Assessment–Feedback Cycle آموزش پزشکی نامه به سردبیر Letter to Editor چکیده کیفیت مستندسازی داده‌های پزشکی یکی از ارکان اساسی ایمنی بیمار، آموزش بالینی مؤثر، تداوم مراقبت و پاسخ‌گویی حرفه‌ای پزشکان محسوب می‌شود. با وجود اهمیت این موضوع، شواهد نشان می‌دهد که مستندسازی پرونده‌های پزشکی در بسیاری از مراکز آموزشی_درمانی همچنان با نواقص ساختاری و کیفی قابل توجهی همراه است؛ نواقصی که می‌توانند بر تصمیم‌گیری‌های بالینی، کیفیت آموزش، ارتباط بین اعضای تیم درمان و حتی پیامدهای حقوقی تأثیر منفی بگذارند. در بسیاری از برنامه‌های آموزشی نیز آموزش مستندسازی به جلسات آموزشی محدود یا دستورالعمل‌های نوشتاری خلاصه می‌شود و معمولاً با ارزیابی مستمر عملکرد و بازخورد ساختاریافته همراه نیست؛ موضوعی که اثربخشی این آموزش‌ها را محدود می‌کند. هدف از این نوشتار، جلب توجه جامعه پزشکی و سیاست‌گذاران آموزشی به ضرورت تغییر رویکرد آموزش مستندسازی از آموزش‌های مقطعی به یک فرایند مستمر، مبتنی بر ارزیابی عملکرد و بازخورد منظم است. همچنین تجربه بیش از ده سال اجرای یک برنامه بهبود کیفیت آموزشی در یکی از بیمارستان‌های آموزشی شمال‌غرب کشور به همراه چالش‌های مرتبط ارائه می‌شود؛ برنامه‌ای که در پاسخ به نواقص تکرارشونده مستندسازی پرونده‌های پزشکی طراحی شد و بر پایه چرخه منسجم آموزش، ارزیابی و بازخورد استقرار یافت. این تجربه نشان می‌دهد که ادغام آموزش ساختاریافته، پایش مستمر کیفیت مستندسازی و ارائه بازخورد منظم به دانشجویان، اعضای هیأت علمی و گروه‌های آموزشی، می‌تواند به بهبود دقت، انسجام و استانداردسازی مستندسازی، تقویت نظارت آموزشی و ارتقای فرهنگ حرفه‌ای ثبت داده‌های پزشکی منجر شود. به نظر می‌رسد این تجربه بتواند به‌عنوان الگویی عملی برای توسعه برنامه‌های مشابه در سایر مراکز آموزشی- درمانی مورد استفاده قرار گیرد. Abstract The quality of medical documentation is a fundamental component of patient safety, effective clinical education, continuity of care, and physicians’ professional accountability. Despite its recognized importance, evidence indicates that medical documentation in many teaching hospitals continues to exhibit substantial structural and qualitative deficiencies. Such shortcomings may adversely affect clinical decision-making, the quality of medical education, communication among healthcare professionals, and even medicolegal outcomes. Furthermore, in many medical curricula, documentation training is largely limited to introductory sessions or written guidelines and is rarely accompanied by continuous performance assessment and structured feedback, thereby limiting its educational effectiveness. This letter aims to highlight the need for a paradigm shift in the way medical documentation is taught from isolated educational interventions to a longitudinal, competency-based process supported by continuous assessment and regular feedback. It also describes the experience of implementing a quality improvement initiative over more than a decade in a teaching hospital in northwestern Iran, together with the challenges encountered during its implementation. The program was developed in response to recurring deficiencies in clinical documentation and was built around an integrated education–assessment–feedback cycle. This experience suggests that integrating structured education with continuous monitoring of documentation quality and systematic feedback to students, faculty members, and academic departments can improve the accuracy, consistency, and standardization of medical documentation while strengthening educational supervision and fostering a professional culture of clinical documentation.
