fa تأثیر اجرای طرح تحول نظام سلامت بر پرداخت از جیب بیماران در بیمارستانهای دولتی منتخب شهر اصفهان عمومى General پژوهشي Research <p dir="RTL"><strong>زمینه:</strong> تأمین عادلانه منابع مالی، در نظام های سلامت به معنی آن است که خطراتی که خانوار به دلیل هزینه های مربوط به نظام سلامت با آن مواجه می شود، برحسب توان پرداخت آنها توزیع شود تا خطر خود بیماری. یک نظام با تأمین مالی عادلانه، از حفاظت همه افراد از نظر مالی، اطمینان حاصل می نماید<span dir="LTR">.</span> افزایش تصاعدی هزینه‌های بخش سلامت در کشور باعث افزایش پرداخت از جیب مردم می گردد. اولین بسته خدمتی در طرح تحول نظام سلامت، برنامه کاهش میزان پرداختی بیماران بستری در بیمارستان‌های وابسته به وزارت بهداشت درمان و آموزش پزشکی می باشد. در این مطالعه به بررسی میزان تحقق این برنامه می پردازیم.</p> <p dir="RTL"><strong>روش کار</strong>: این پژوهش، با بررسی اطلاعات درآمدی 5 بیمارستان دولتی شهر اصفهان وابسته به دانشگاه علوم پزشکی و خدمات بهداشتی درمانی که شامل عمومیت تخصص ها بود، در 9 ماه ابتدای اجرای طرح صورت گرفته است.</p> <p dir="RTL"><strong>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; یافته ها</strong>: با احتساب نرخ تورم، به طور متوسط میزان پرداخت از جیب بیماران <strong>با میانگین 18/2 و انحراف معیار 82/1 و نهایتا، 17/43 درصد کاهش داشت</strong> که می تواند به دلیل پرداخت یارانه سلامت از جانب دولت باشد.</p> <p dir="RTL">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; <strong>نتیجه گیری</strong>: تحلیل یافته ها نشان داد که دولت توانسته میزان پرداخت از جیب بیماران را به میزان قابل توجهی کاهش دهد و گامی موثر در راستای رسیدن به اهداف خویش بردارد. اما بررسی علل افزایش هزینه ها و میزان بار مراجعات ضروری به نظر می رسد.</p> <p dir="RTL"><strong>واژگان کلیدی:</strong> طرح تحول نظام سلامت، بیمارستان، پرداخت از جیب</p> <p>Providing financial resources in a justly manner in health care system means, that paying</p> <p>Health care services by patients must be based on their financial ability and not by severity of their condition or danger of their disease. In a fair health care system, protection of all patients in term of their financial ability has been predicted. On the other hand, exponential increases in cost of health services have been lead to paying more out of patient&rsquo;s pocket. In the first pack of the new Health Care Reform Plan, the program will reduce the amount paid by patients admitted in the hospitals of the ministry of health and medical education .in this study we are going to deal with the realization of the mentioned plan.</p> <p>Methodology:</p> <p>This study was done in the first 9 months of the new plan, by gathering income data of 5 government &ndash;affiliated hospitals&nbsp;&nbsp; that are related to Isfahan Medical University.</p> <p>Findings:</p> <p>With considering the inflation rate, the average rate of paying out of pocket was reduced by 43.17% with mean of 2.18 and Standard Deviation of 1.82. This could be due to paying subsidiaries for health care by government.</p> <p>Conclusion:</p> <p>Analyzing the data shows that governed has taken big steps in reducing the amount of health care costs</p> <p>Out of patient&rsquo;s pocket.
