fa مقایسهای استفاده از کوتر و اسکالپل در برشهای جراحی شکم میدلاین در بیماران مراجعهکننده به بیمارستان لقمان حکیم بین سال 1391 تا 1392 عمومى General پژوهشي Research <p dir="RTL"><strong>مقدمه:</strong> سال‌ها برش جراحی توسط اسکالپل انجام می شده است و اکنون استفاده از کوتر بعنوان جایگزین اسکالپل روز به روز رو به افزایش است. هر کدام از این روش ها داری معایب و مزایایی هستند. هدف از انجام این مطالعه مقایسه این دو روش در ایجاد برش جراحی در لاپاراتومی میدلاین می باشد.</p> <p dir="RTL"><strong>روش مطالعه:</strong> ۱۰۰ بیمار که طی سال ۱۳۹۱ لغایت ۱۳۹۲ تحت لاپاراتومی میدلاین قرار گرفتند و معیارهای ورود به مطالعه را داشتند به دو گروه تقسیم شدند. در یک گروه از اسکالپل و در گروه بعدی از کوتر استفاده شد. سپس سرعت ایجاد برش جراحی، میزان خونریزی، درد پس از جراحی و عوارض بوجود آمده در زخم با هم مقایسه شدند.</p> <p dir="RTL"><strong>یافته ها:</strong> در این مطالعه میانگین و انحراف معیار سرعت ایجاد برش جراحی بر مبنای سانتی متر بر دقیقه محاسبه شد که در روش کوتر این میزان 3.052 &plusmn; 10.64 و در روش اسکالپل 3.61 &plusmn; 8.26 بدست آمد. میانگین و انحراف معیار میزان خونریزی نیز بر مبنای تعداد گاز مصرفی خونی شده در گروه کوتر 0.37 &plusmn; 0.97 و در گروه اسکالپل 1.25 &plusmn; 2.26 گاز بود. همچنین از بین افرادی که در آنها از کوتر استفاده شد %8.2 و از بین افرادی که برای آن&rlm;ها از اسکالپل استفاده شد %11.8 به عارضه زخم مبتلا شدند.</p> <p dir="RTL"><strong>بحث و نتیجه گیری:</strong> استفاده از کوتر سرعت جراحی را افزایش و میزان خونریزی را کاهش می دهد. در حالی که میزان درد پس از جراحی و عوارض بوجود آمده در زخم بین دو گروه تفاوت معناداری با یکدیگر نداشتند. بنابراین استفاده از کوتر به عنوان روشی آسان و مناسب جهت برش پوستی و دایسکشن بافتی می تواند جایگزین اسکالپل باشد.</p> <p dir="RTL"><strong>واژگان کلیدی: </strong>برش جراحی، کوتر، اسکالپل</p> <p></p> <p>Introduction :</p> <p>For many years surgical incisions were done by scalpel. Nowadays this method has been replaced with</p> <p>Electrocautery.Each of these methods has their disadvantages and benefits. The purpose of this study is to compare these two methods in midline laparotomy surgeries.</p> <p>Methods :</p> <p>&nbsp;From 2012 until 2013, One hundred patients enter the study, underwent midline laparotomy and were divided into two Groups .In one group scalpel and in the other group Electrocauterywere&nbsp; used .</p> <p>The speed of surgical resection, bleeding, postoperative pain and other wound complications were compared between two groups.</p> <p>Finding :</p> <p>In this study, the mean and standard deviation speed was calculated based on centimeter per minutes .</p> <p>In Electracautery group result was 10.64+_3.052 and in scalpel group was 8.26+_3.61.The average Andstandard deviation of the bleeding based on number of bloody used gauzes in Electracautery group was .97+_.37 and in other group was 2.26+_1.25.Wound complications among the patients with Electrocauter usage were 8.2% versus 11.8% in the other group.</p> <p>Discussion and conclusion:</p> <p>Amount of pain and wound complication were similar in two groups .However, using Electrocautery device increases the speed of operation and decreases bleeding amount .Therefore ,surgical scalpel can be replaced by Electrocauter.</p> 0 0 http://jmciri.ir/browse.php?a_code=A-10-1-1662&slc_lang=fa&sid=1
