fa ضرورت حفظ و تکامل نظام یکپارچه درمان اعتیاد و کاهش آسیب در ایران؛ نگاهی به آینده نظام سلامت The Necessity of Preserving and Advancing the Integrated Addiction Treatment and Harm Reduction System in Iran: A Look Toward the Future of the Healthcare System سیاست گذاری سلامت نامه به سردبیر Letter to Editor در دو دهه‌ی گذشته، ایران دستاوردهای قابل توجهی در کنترل پیامدهای بهداشتی و اجتماعی مصرف مواد و به‌ویژه مهار انتقال عفونت‌های مرتبط با مصرف تزریقی به دست آورده است. این موفقیت که در مقایسه با بسیاری از کشورهای منطقه کم‌نظیر محسوب می‌شود، حاصل شکل‌گیری سیاست‌گذاری منسجم مبتنی بر رویکرد سلامت عمومی و نگاه سیستمی به پدیده‌ی اعتیاد بوده است (1). در این چارچوب، درمان دارویی، مداخلات روان‌شناختی و برنامه‌های کاهش آسیب در قالب یک ساختار یکپارچه و عمدتاً در بستر مراکز درمان سوءمصرف مواد ارائه شده‌اند. تجربه عملی این سال‌ها نشان داده است که اثربخشی درمان اعتیاد نه صرفاً به دارو، بلکه به بسته‌ی خدماتی جامع ارائه شده در این مراکز که شامل پایش پزشکی، حمایت روانی-اجتماعی و ارتباط مستمر با نظام سلامت وابسته است. در ادبیات نوین پزشکی نیز مفهوم "درمان" دستخوش تحول شده است. در بسیاری از بیماری‌های مزمن، درمان دیگر به معنای بازگشت کامل به وضعیت پیش از بیماری تعریف نمی‌شود، بلکه به عنوان فرآیندی برای کاهش بار بیماری و مدیریت پایدار آن شناخته می‌شود. اعتیاد نیز به عنوان یک اختلال مزمن عودکننده در همین چارچوب قابل فهم است. در این رویکرد، هدف درمان کاهش شدت و پیامدهای بیماری، کاهش رفتارهای پرخطر، بهبود کارکرد اجتماعی و تثبیت وضعیت فرد است؛ حتی اگر حذف کامل بیماری در کوتاه‌مدت امکان‌پذیر نباشد. درمان‌های نگهدارنده با آگونیست‌هایی مانند متادون و بوپرنورفین، در کنار مداخلات روان‌شناختی و اجتماعی، دقیقاً با همین هدف طراحی شده‌اند تا با کاهش ولع مصرف و تثبیت وضعیت زیستی و رفتاری بیمار، امکان دستیابی به یک تعادل پایدار در زندگی فرد فراهم شود. مطالعات انجام‌شده در ایران نیز مؤید اثربخشی این رویکرد هستند. پژوهش‌هایی مانند مطالعه‌ی سیدعلینقی و همکاران نشان داده‌اند که گسترش درمان‌های نگهدارنده همراه با برنامه‌های کاهش آسیب نظیر توزیع سرنگ استریل، مراکز گذری و ادغام خدمات پیشگیری از HIV با درمان اعتیاد، نقش مهمی در کاهش رفتارهای تزریقی پرخطر و در نتیجه مهار انتقال HIV در میان مصرف‌کنندگان مواد داشته است (2). این مداخلات توانسته‌اند فراوانی تزریق و استفاده مشترک از سرنگ را کاهش داده و در نتیجه بار عفونت‌های منتقله از راه خون را به طور معناداری کنترل کنند. تجربه ایران در این زمینه، در مقایسه با بسیاری از کشورهای همسایه که عمدتاً بر رویکردهای مبتنی بر کاهش عرضه و برخورد انتظامی تمرکز داشته‌اند، نشان می‌دهد که سیاست‌های مبتنی بر سلامت عمومی می‌توانند به ثبات الگوی مصرف و کاهش آسیب‌های اجتماعی منجر شوند. Over the past two decades, Iran has achieved considerable progress in reducing the health and social consequences of substance abuse. Compared with many countries in the region, it has been relatively successful in controlling blood borne infections among people who inject drugs. This achievement has largely resulted from coherent public health policies and the expansion of a nationwide network of substance abuse treatment centers. Experience has shown that effective addiction treatment depends not only on pharmacotherapy, but also on integrating medication with clinical, psychological, and social services delivered through these centers. Addiction is widely recognized as a chronic relapsing disorder, and treatment does not necessarily imply complete elimination of the disease. Rather, the primary goals are to reduce its severity and consequences, minimize high risk behaviors, and improve social functioning. Within this framework, opioid agonist maintenance treatments such as methadone and buprenorphine, combined with psychosocial interventions, reduce craving and stabilize patients’ biological and behavioral conditions, enabling them to achieve a more stable and functional life. Studies in Iran have shown that expanding these treatments, together with harm reduction measures such as sterile syringe distribution, drop in centers, and integration of HIV prevention with addiction treatment, has significantly reduced high risk injecting behaviors and helped control HIV transmission among people who use drugs. A key strategy for improving service quality and efficiency is the implementation of center stratification and a stepped-care model. In this approach, services are organized according to disorder severity, comorbidities, and patients’ biological, psychological, and social needs. Such a structure promotes specialization, closer monitoring, and more effective referral pathways while facilitating the gradual expansion of pharmacological options and the controlled introduction of specialized therapies. Overall, Iran’s experience demonstrates that successful addiction management relies on an integrated model combining pharmacotherapy, psychosocial services, and health monitoring within treatment centers.
