fa اطلاعات دموگرافیک و بالینی افراد مبتلا به سرطان ریه سلول غیر کوچک و ارتباط آن با نرخ جهش های ژن EGFR در جمعیت ایرانی Demographic and clinical information of patients with non-small cell lung cancer and its relationship with EGFR gene mutation rates in Iranian population عمومى General پژوهشي Research چکیده زمینه: جهش های ژن گیرنده فاکتور رشد اپیتلیالی (EGFR) عمدتاً در اگزون های 18 تا 21 مشاهده می گردد. بررسی موتاسیون‌ها در روند پاسخ به درمان بیماران مبتلا به سرطان ریه سلول غیر کوچک (NSCLC) نسبت به داروهای مهارکننده‌ تیروزین کینازی بسیار حائز اهمیت بوده و فراوانی جهش‌های ژن EGFR در بیماران مبتلا به NSCLC در مناطق جغرافیایی مختلف جهان متفاوت است. هدف مطالعه حاضر بررسی شایع ترین جهش‌های ژن EGFR بیماران مبتلا به سرطان ریه درجمعیت ایرانی می‌باشد. روش کار: در این مطالعه اگزون های 18 تا 21 ژن EGFR 160 بیمار مبتلا به NSCLC مورد بررسی قرار گرفت. بدین منظور ابتدا بر روی DNA استخراج شده از بلوک پارافینی، PCR و سپس تعیین توالی انجام و نتایج آنها مورد تجزیه و تحلیل قرارگرفت. یافته ها: نسبت شیوع NSCLC بیماران مرد به زن حدود 2 به 1 بود و تفاوت چندانی از نظر سن در بیماران مرد و زن وجود نداشت (P=0/065) در حالی که در متغیر سیگار کشیدن اختلاف قابل ملاحظه ایی یافت شد (مردان %77/8 و زنان %17/3 ). میزان فراوانی برای جهش های اگزون های 19 و 21 ژن EGFR، برابر با %10/63 و برای اگزون‌های 18 و 20 هیچ جهشی مشاهده نشد. از طرفی زنان در مقایسه با مردان نسبت به جهش‌های EGFR حساس‌تر بوده ( %17/3  در مقابل %7/4) اما از نظر آماری مقادیر بدست آمده معنی دار نبود (P=0/097). در مطالعه حاضر 3 جهش شامل حذف‌های (c.2235_2249del) و (c.2240_2257del) در اگزون 19 و جهش نقطه ای (c.2573T>G) p.L858R  در اگزون 21 به ترتیب با فراوانی%58/8، %23/5 و %11/8 مشاهده شد. همچنین، یک جهش جایگزینی c.2253A>G  برای اولین بار به عنوان یک جهش جدید یافت شد که نادرترین جهش با فراوانی %5/8 بود. نکته قابل توجه مشاهده یک رابطه منفی بین میزان موتاسیون در ژن EGFR و سیگاری بودن افراد مبتلا می‌باشد که با توجه به بررسی سایر مطالعات کشورهای مختلف می‌توان تنوع در قومیت افراد را به عنوان علت این یافته ها فرض کرد. نتیجه گیری: وقوع جهش EGFR تا حد زیادی در بین بخش های مختلف ایران متفاوت است، که احتمالاً به دلیل تفاوت در قومیت، میزان مصرف سیگار و جنسیت بیماران می باشد. Abstract Background: Mutations in epidermal growth factor receptor (EGFR) gene are mainly observed in exons 18 to 21. Evaluation of mutations in the response to treatment of patients with non-small cell lung cancer (NSCLC) is more important than tyrosine kinase inhibitors (TKIs) and the frequency of EGFR gene mutations varies in NSCLC patients in different geographical regions of the world. Methods:  In this study, exons 18 to 21 of the EGFR gene were studied in 160 patients with NSCLC. For this purpose, DNA was extracted from paraffin block, PCR and then sequencing was performed and the results were analyzed. Results: The prevalence ratio of NSCLC in male to female patients was about 2 to 1 and there was no significant difference in age between male and female patients (P = 0.065), while a significant difference was found in the smoking variable (77.8 men and 17.3% women). The frequency of mutations for exons 19 and 21 of EGFR gene was 10.63% and no mutations were found for exons 18 and 20. On the other hand, women were more sensitive to EGFR mutations than men (17.3% versus 7.4%), however this difference was not statistically significant (P=0.097). In this study, 3 mutations including (c.2235_2249del) and (c.2240_2257del) in exon 19 and (c.2573T> G) p. L858R in exon 21 were observed with frequencies of 58.8, 23.5 and 11.8%, respectively. Also, an alternative mutation including (c.2253 A> G) was found as a new mutation for the first time, which was the rarest mutation with a frequency of 5.8%. Interestingly, a negative relationship was observed between the mutation rate in EGFR gene and smoking of infected people that according to other studies in different countries, diversity in ethnicity can be assumed as the cause of these findings.
