fa مقایسه اثربخشی ورزش هوازی با و بدون ذهن‌آگاهی بر قندخون ناشتا و افسردگی در زنان مبتلا به دیابت نوع 2 The Effectiveness of Aerobic Exercise with or without Mindfulness on Fasting Blood Sugar and Depression in Women with Type 2 Diabetes بیماری های غدد پژوهشي Research چکیده زمینه: شیوع دیابت جهانی در حال افزایش است. همراهی دیابت و افسردگی به پیچیدگی مدیریت بیماری می‌افزاید. تمرکز مطالعات جدید برروی یافتن راهی برای بهبود مشکلات روانی و افزایش انگیزه بیماران برای انجام و ادامه ورزش و دستورات پزشکی است. ذهن آگاهی به دلیلتمرکز بر الگوهای فکری موثر بر بیماری مورد توجه ویژه قرار گرفته است. پژوهش حاضر با هدف مقایسه اثربخشی ورزش هوازی با و بدونذهنآگاهی بر قندخون ناشتا و افسردگی در زنان مبتلا به دیابت نوع 2 انجام شد. روش کار: طرح پژوهش حاضر، نیمه آزمایشی همراه با پیش آزمون-پس آزمون و پیگیری سه ماهه بود. جامعه آماری پژوهش را زنان مبتلا بهدیابت نوع دو مراجعه کننده به مرکز دیابت در سال 1402 در شهر کرج تشکیل می‌دادند که با ‌روش نمونه‌گیری در دسترس انتخاب و به شیوهتصادفی در چهارگروه 15 نفری قرار گرفتند. گروه ورزش هوازی علاوه بر درمان پزشکی 12 جلسه تمرینات، گروه دوم 12 جلسه مداخله ذهنآگاهی و گروه سوم نیز ترکیب ورزش هوازی و ذهن آگاهی دریافت کردند. گروه کنترل فقط درمان پزشکی داشتند. 4 گروه در مراحل پیش‌آزمون،پس‌آزمون و پیگیری به پرسشنامه افسردگی بک پاسخ دادند و سطح قندخون ناشتا سنجیده شد. تجزیه و تحلیل با آزمون تحلیل واریانس بااندازه‌گیری مکرر و آزمون تعقیبی بنفرونی انجام شد. یافته‌ها: طبق یافته‌ها قبل از مداخله بین گروه ها تفاوت معناداری وجود نداشت. بعد از مداخله گروه ترکیبی ورزش هوازی-ذهن‌آگاهی تاثیربیشتری بر قندخون ناشتا و افسردگی داشتند. بین اثربخشی ورزش هوازی و ترکیب ورزش هوازی/ ذهن آگاهی (0/003=p) و ذهن آگاهی وترکیب ورزش هوازی/ ذهن آگاهی (0/015=p) بر افسردگی و قند خون ناشتای بیماران مبتلا به دیابت نوع 2 تفاوت معنادار وجود داشت. نتیجه‌گیری: می‌توان نتیجه گرفت ترکیب ورزش هوازی-ذهن‌آگاهی تاثیر ایده آلی در کاهش قند خون ناشتا و افسردگی در زنان مبتلا بهدیابت نوع 2 دارد. این روش می‌تواند در مداخلات درمانی دیابت نوع 2 در اولویت قرار گیرد. اگرچه درمان ترکیبی نتایج امیدوارکننده‌ای نشانداد اما کارآزمایی‌های بالینی بیشتری برای ارزیابی اثربخشی در جوامع مختلف و سایر وضعیت‌های روانی همزمان مورد نیاز است. Abstract Background: The global diabetes prevalence is rising.The comorbidity of diabetes and depression add complexity to diabetes management. The focus of new studies is on finding a way to improve psychological problems and increase the motivation of patients to perform and continue exercise and medical orders. The mindfulness has attracted special attention due to its focus on the thought patterns and emotions of people that can play an important role in controlling diabetes. This study aimed to compare the effectiveness of aerobic exercise with or without mindfulness in fasting blood sugar levels and depression among women with type 2 diabetes. Methods: This study employed a semi-experimental design featuring a pre-test, post-test, and a three-month follow-up. The statistical population consisted of women with type 2 diabetes who were referred to Diabetes Center in 2023 in Karaj city. 60 participants were randomly selected and divided into four groups, with 15 individuals in each group. In addition to medical treatment, the aerobic exercise group participated in 12 sessions of exercises, while the second group underwent 12 sessions of mindfulness intervention. The third group received both 12 sessions of mindfulness and 12 sessions of aerobic exercises. The control group received only medical treatment. All four groups completed the Beck Depression Inventory during the pre-test, post-test, and follow-up phases, their fasting blood sugar levels were measured. Data were analyzed using repeated measures analysis of variance (ANOVA) and Bonferroni post hoc tests. Results: The results revealed significant differences in the effectiveness of aerobic exercise compared to the combined aerobic exercise/mindfulness intervention (p=0.003), as well as between mindfulness therapy and the combined intervention (p=0.015) in reducing depression and fasting blood sugar levels in patients with type 2 diabetes. The combination of aerobic exercise and mindfulness was more effective in reducing both depression and fasting blood sugar. Conclusion: The combination of aerobic exercise and mindfulness training has an ideal therapeutic effect in reducing fasting blood sugar levels and depression on patients with type 2 diabetes.
