fa بررسی مقایسه ای سه ترکیب دارویی جهت اعمال بلوک عصبی انگشت در بیماران با آسیب انگشتان دست Comparative Study of Three Drug Combinations for Digital Nerve Blocks in Patients with Finger Injuries سایر موارد پژوهشي Research چکیده زمینه: آسیب‌های انگشت از جمله شکستگی، دررفتگی و پارگی در پوست، اعصاب، رگ‌های خونی و یا تاندون‌ها، از شایع‌ترین آسیب‌های ارتوپدی هستند که نیاز به مدیریت درد مؤثر دارند. هدف از این مطالعه، ارزیابی و مقایسه سه ترکیب دارویی مختلف شامل لیدوکائین به تنهایی، لیدوکائین همراه با دگزامتازون و لیدوکائین همراه با بیکربنات‌سدیم در ایجاد بلوک عصبی انگشتان برای بیماران ترومایی بود. این مطالعه با هدف تعیین بهترین ترکیب دارویی برای کاهش درد، افزایش مدت زمان بی‌دردی و بهبود رضایت بیماران انجام شد.           روش کار: این مطالعه توصیفی تحلیلی گذشته‌نگر بر روی 90 بیمار بین سنین 18 تا 90 سال انجام شد که در سال 1397 به بیمارستان امام رضا (ع) تهران مراجعه کرده بودند. بیماران به سه گروه تقسیم شدند: گروه A (لیدوکائین به تنهایی)، گروه B (لیدوکائین+دگزامتازون)، و گروه C (لیدوکائین+بیکربنات سدیم). داده‌های جمع‌آوری شده شامل جنسیت، شاخص توده بدنی (BMI)، زمان شروع بی‌دردی، مدت زمان بی‌دردی، رضایت بیماران و شدت درد پس از عمل جراحی بود. تحلیل داده‌ها با استفاده از نرم‌افزار SPSS و آزمون‌های آماری انجام شد.         یافته‌ها: نتایج نشان داد که گروه B (لیدوکائین+دگزامتازون) بیشترین رضایت بیماران (%70) و کمترین شدت درد (%76/7 درد خفیف) را داشت. همچنین، این گروه طولانی‌ترین مدت زمان بی‌دردی (96/71 دقیقه) را نشان داد. در مقابل، گروه C (لیدوکائین+بیکربنات سدیم) سریع‌ترین زمان شروع بی‌دردی (2/49 دقیقه) را داشت، اما مدت زمان بی‌دردی در این گروه کوتاه‌تر بود (37/07 دقیقه). گروه A (لیدوکائین به تنهایی) کمترین رضایت بیماران (%16/7) و بیشترین شدت درد (%66/7 درد متوسط) را داشت. تفاوت‌ها بین گروه‌ها از نظر آماری معنی‌دار بود (P<0/001).                نتیجه‌گیری: ترکیب لیدوکائین با دگزامتازون به عنوان یک روش مؤثر در بلوک عصبی انگشتان، نه تنها درد را به طور قابل توجهی کاهش داد، بلکه رضایت بیماران را نیز افزایش داد و مدت زمان بی‌دردی را به طور معنی‌داری طولانی‌تر کرد. این یافته‌ها می‌تواند به بهبود پروتکل‌های مدیریت درد در بیماران ترومایی کمک کرده و استفاده از دگزامتازون به عنوان یک مکمل مؤثر در بلوک‌های عصبی را توجیه ‌کند. Abstract Background: Trauma of the finger, such as fracture, dislocation, and laceration of skin, nerves, vessels, or tendons, is one of the most frequent orthopedic traumas requiring proper pain relief. The objective of this study was to compare and assess three drug combinations lidocaine only, lidocaine plus dexamethasone, and lidocaine plus sodium bicarbonate for digital nerve blocks in patients with traumatic finger injuries. The investigation was modeled to determine the best combination of drugs to minimize pain, prolong the period of analgesia, and achieve maximum patient satisfaction. Methods: This was a retrospective descriptive-analytical study of 90 hospitalized patients aged between 18 to 90 years, who were hospitalized in Imam Reza Hospital, Tehran, Iran, during the year 2018. The patients were divided into three groups: Group A (lidocaine single injection), Group B (lidocaine+dexamethasone), and Group C (lidocaine+sodium bicarbonate). Data gathered were gender, body mass index (BMI), time of onset of analgesia, duration of analgesia, patient satisfaction, and intensity of postoperative pain. Data were analyzed statistically by SPSS software and suitable statistical tests. Results: The findings showed that Group B (lidocaine+dexamethasone) showed the maximum patient satisfaction rate (70%) and minimum severity of pain (76.7% with mild pain). The duration of analgesia was also maximum in the same group (96.71 minutes). However, Group C (lidocaine+sodium bicarbonate) presented with the fastest onset of analgesia (2.49 minutes) but with the shortest duration of analgesia (37.07 minutes). Group A (lidocaine) also presented with the least patient satisfaction (16.7%) and the greatest degree of pain (66.7% moderate pain). The comparison between the groups was statistically different (P<0.001). Conclusion: The addition of dexamethasone to lidocaine was found to be a very effective way of digital nerve blocks, decreasing pain significantly, improving patient satisfaction, and even the duration of analgesia.
