fa نقشی فراتر از تشخیص: اهمیت پیش‌بینی‌کننده رادیوگـرافی بینی در موفقیت درمان شکستگی‌های بینی Beyond Diagnosis: The Predictive Role of Nasal Radiography in the Treatment Success of Nasal Bone Fractures بیماری های گوش و حلق و بینی نامه به سردبیر Letter to Editor چکیده آسیب‌های بینی از شایع‌ترین علل مراجعه بیماران به بخش‌های اورژانس گوش، گلو و بینی در مراکز درمانی کشور ایران هستند. با توجه بهحجم بالای این مراجعات، انتخاب صحیح روش تشخیص و درمان می‌تواند تأثیر قابل توجهی در کاهش عوارض، بهبود نتایج درمانی و افزایشرضایت بیماران داشته باشد. در مقاله «مقایسه بین یافته‌های رادیولوژی و بالینی در تشخیص شکستگی بینی» (1)، که در مجله علمی سازمان نظام پزشکی جمهوریاسلامی ایران منتشر شده است، نویسندگان به بررسی ارزش رادیوگرافی در تشخیص شکستگی بینی پرداخته‌اند. از میان 174 بیمار باتشخیص بالینی شکستگی، 131 نفر (٪81) دارای یافته‌های مثبت رادیولوژی بودند و 33 نفر (٪19) با وجود علائم بالینی، رادیوگرافی منفیداشتند. همچنین ٪26 بیماران دارای اکیموز اطراف چشمی بودند. این نتایج نشان‌دهنده محدودیت نسبی رادیوگرافی در تشخیص اولیهشکستگی بینی بود. در مطالعه‌ای که توسط همکاران اینجانب در دانشگاه علوم پزشکی تهران انجام شده و در نشریه British Journal of Oral and MaxillofacialSurgery <img align="left" alt="Text Box: (2)،" src="data:image/png;base64,iVBORw0KGgoAAAANSUhEUgAAABAAAAAWCAYAAADJqhx8AAAAAXNSR0IArs4c6QAAAHhlWElmTU0AKgAAAAgABQESAAMAAAABAAEAAAEaAAUAAAABAAAASgEbAAUAAAABAAAAUgEoAAMAAAABAAIAAIdpAAQAAAABAAAAWgAAAAAAAACQAAAAAQAAAJAAAAABAAKgAgAEAAAAAQAAABCgAwAEAAAAAQAAABYAAAAAHv5cGgAAAAlwSFlzAAAWJQAAFiUBSVIk8AAAAEJJREFUOBFjFBEROScIBAxkgPdAwALSfOvWLQUy9DOoqakxMJGjEVnPqAEMo4EITBAUpwNGSvMCcqocZY+GANkhAABwUQ2TGcjtqwAAAABJRU5ErkJggg==" style="width:19px; height:16px" >منتشر گردیده، 80 بیمار با شکستگی بینی تحت جااندازی بسته قرار گرفتند که از این میان، 56 نفر (٪70) با موفقیت درمانشدند. تحلیل آماری نشان داد که نبود خط شکستگی در رادیوگرافی لترال بینی با عدم موفقیت در جااندازی ارتباط معناداری داشت(p=0/000). همچنین وجود اکیموز اطراف چشم با موفقیت جااندازی مرتبط بود (p=0/042). نکته‌ای که در بسیاری از مراکز درمانی کشور مشاهده می‌شود، انجام رادیوگرافی لترال بینی به عنوان یک اقدام قانونی و جهت تکمیل پروندهپزشکی است، نه صرفاً برای تشخیص بالینی. با این حال، دشواری تفسیر تصاویر رادیولوژی بینی به‌ویژه به دلیل تنوع آناتومیک و محدودیتوضوح شکستگی‌ها اغلب موجب سردرگمی بیماران و حتی تردید در تصمیم‌گیری درمانی می‌شود. با توجه به یافته‌های فوق، پیشنهاد می‌شود که رادیوگرافی بینی نه صرفاً به عنوان ابزار تشخیصی، بلکه به عنوان ابزاری برای پیش‌بینیموفقیت درمان نیز مدنظر قرار گیرد. دیدن خط شکستگی در رادیوگرافی لترال بینی می‌تواند به عنوان شاخصی برای انتخاب مناسب‌ترین روشدرمانی یعنی انجام یا عدم انجام جااندازی بسته و نیز میزان موفقیت روش درمانی انتخابی مورد استفاده قرار گیرد. این رویکرد می‌تواند بهانتخاب دقیق‌تر بیماران برای مداخله درمانی و کاهش موارد جااندازی ناموفق کمک شایانی نماید.   Abstract Nasal trauma is one of the most frequent causes of patient visits to otolaryngology emergency departments across healthcare centers in Iran. Given the high volume of such cases, selecting the appropriate diagnostic and therapeutic approach can significantly reduce complications, improve treatment outcomes, and enhance patient satisfaction. In the article titled “Comparison Between Radiological and Clinical Findings in the Diagnosis of Nasal Bone Fractures” [1], published in the Scientific Journal of the Iranian Medical Council, the authors evaluated the diagnostic value of radiography in nasal bone fractures. Among 174 patients with a clinical diagnosis of nasal fracture, 131 (81%) had positive radiographic findings, while 33 (19%) showed negative radiographs despite clinical signs. Additionally, 26% of patients presented with periorbital ecchymosis. These findings highlight the relative limitations of radiography in the initial diagnosis of nasal fractures. In a separate study conducted by my colleagues at Tehran University of Medical Sciences and published in the British Journal of Oral and Maxillofacial Surgery [2], 80 patients with nasal fractures underwent closed reduction. Of these, 56 patients (70%) were successfully treated. Statistical analysis revealed a significant association between the absence of a visible fracture line on lateral nasal radiographs and unsuccessful reduction (p = 0.000). Furthermore, the presence of periorbital ecchymosis was significantly associated with successful reduction (p = 0.042). A common observation in many Iranian healthcare centers is the routine use of lateral nasal radiography primarily for legal documentation and medical record completion, rather than for clinical decision-making. However, interpreting nasal radiographs is often challenging due to anatomical variability and the limited clarity of fracture lines, which can lead to confusion among patients and even hesitation in clinical decision-making. Based on the above findings, it is recommended that nasal radiography be considered not only as a diagnostic tool but also as a predictor of treatment success. The presence of a visible fracture line on lateral nasal radiographs may serve as a valuable indicator for selecting the most appropriate treatment strategy—namely, whether or not to perform closed reduction—and for estimating the likelihood of therapeutic success.
